Specialist Nursing Services
Managing your diabetes in partnership with the RDNS Team of Diabetes Educators’
RDNS Diabetes Educators work in partnership with General Practitioners, Endocrinologists, general medical specialists, hospitals and community services in assisting and supporting people in the management of type 2 or type 1 Diabetes.
Our aim is to provide services to people who are newly diagnosed or who are experiencing challenges with any aspect of their diabetes management.
RDNS has a team of (4) advanced clinicians, (3) of whom are Credentialed Diabetes Educators. We provide the following services:
- Individualised comprehensive diabetes assessment
- Development of a negotiated diabetes health care plan
- Education and clinical support relating to an individual’s needs and their goals including,
- Dietary challenges
- Exercise and life style changes
- Blood glucose monitoring
- Medication management
- commencement of insulin therapy
- Services for children, their families, schools and child care services
Community Programs and Services
The Diabetes Team provides several community based programs in partnership with other organisations:
- Taoundi Aboriginal Diabetes Group – a fortnightly education and support group including a healthy lunch based at Taoundi College Port Adelaide
- Aldinga GP Plus – Initiation of Insulin and Diabetes Education Services – in partnership with the Southern Adelaide Health Service
- Stepping Away from Diabetes –a healthy lifestyle program for people at risk of Diabetes. (funded through the Australian Department of Health and Aging from August 2008 – December 2009)
- Provision of Medicare funded diabetes education services in collaboration with General Practitioners
- Education to schools, health services and community groups
