Transition: moving through life’s adverse disruptions

Dr Debbie Kralik and Dr Antonia van Loon recently developed a poster titled ‘Transition: moving through life’s adverse disruptions’, for the United Nations Conference ‘Engaging Healthy Communities' held during August 2005 in Brisbane. This poster outlines the developing transitions theory that has been the focus of Dr Kralik’s research for the past decade, and more recently Dr van Loon.

The poster includes a (tested) practical model that community health workers can use to assist people to move through adversity. This model has direct applicability to the National Chronic Illness Self Management agenda.
